Our Health Coach Works with You to Maintain or Improve Your Health

Population Health Services:
HCHS has Population-Health based services available to both hospital and clinic patients. While you can be referred by a medical provider, you do not need a referral to access the services of a Health Coach!

HCHS has two components of its Population Health Services: A Diabetic Educator and a Health Coach.

Health coaching provides you with  an opportunity to talk about what is most troubling  about your health conditions, what you most want to change about your health, what  support you need to make these changes, and what obstacles have to be removed to help you improve your health.

With the help of HCHS Population Health, you will be able to set realistic goals and together, you and your Population Health Team, can discuss how you feel about meeting those goals.
